Without an Understanding of Purpose and Context Data is Blind: The Development of a Model of Care Toolkit (#51)
High quality, relevant data is essential to ongoing practice improvement. However data on its own does not change practice. To make the most out of data teams need to have a shared understanding of their purpose. This may seem self evident yet many teams do not have a shared understanding of their model of care in use nor can they articulate clearly what they do and why.
This presentation outlines the development of a Model of Care Toolkit which has been developed to assist multidisciplinary teams to gain better clarity about their purpose and context. It also assists teams to better understand the shape of care and care pathways; foster agreements about best practice; increase transparency within health care provision; take into account the context of care setting and strengthen the outcomes of care.
